血尿酸水平与首次急性脑梗死两年预后的关系

摘要:背景 近年来的流行病学研究表明,血尿酸(Uric acid,UA)水平与急性脑梗死(Acute Ischemic Stroke,AIS)患者的结局有关,但UA对AIS患者是否具有正面影响仍然存在争议。目的 探讨早期不同UA水平是否与首次发生AIS的患者两年内复发及全因死亡情况有关,进一步为制定公共卫生健康教育干预策略提供科学依据。方法 采用回顾性队列研究选取2018年1月1日至2019年12月31日在广东省深圳市某三甲医院神经内科住院的首次发生AIS的患者。收集患者住院期间的一般信息、生化指标及两年内复发和全因死亡情况,通过Logistic回归分析观察卒中时UA水平是否与AIS患者的不良预后有关。结果 不同尿酸水平的患者出院时住院天数、住院花费、出院NIHSS评分、BADL(Basic Activities of Daily Living,BADL)评分及两年内的复发和全因死亡风险总体无统计学差异(P>0.05),但相对于Q1组,Q4组的住院花费显著降低(P 0.05)。结论 相对于较低水平的UA,较高水平的UA与男性AIS患者两年内复发及全因死亡率较低有关。但我们的研究并不意味着高尿酸能降低首次急性脑梗死患者的复发及全因死亡率。

英文摘要:Background Epidemiological studies in recent years have shown that serum uric acid (UA) levels are associated with outcomes in patients with acute cerebral infarction (AIS), but whether UA has a positive effect on patients with AIS remains controversial. Objective To explore whether different early UA levels are related to the recurrence and all-cause death within two years in patients with AIS for the first time, and further provide a scientific basis for the development of public health education intervention strategies. Methods A retrospective cohort study was used to select patients with first AIS who were hospitalized in the Department of Neurology of a tertiary hospital in Shenzhen, Guangdong Province from January 1, 2018 to December 31, 2019. The general information, biochemical indicators, recurrence and all-cause death within two years during hospitalization were collected, and Logistic regression analysis was used to observe whether the level of UA during stroke was related to the poor prognosis of AIS patients. Results There was no statistical difference in the number of days of hospitalization, hospitalization cost et al. between patients with different uric acid levels (P>0.05). However, compared with the Q1 group, the hospitalization cost of the Q4 group was significantly lower (P 0.05). Conclusion Relative to lower levels of UA, higher levels of UA were associated with lower 2-year recurrence and all-cause mortality in male AIS patients. However, our study does not mean that high uric acid can reduce recurrence and all-cause mortality in patients with first acute cerebral infarction.
