  • Study on the Evidence Map of Manual Therapy of Clinical Advantageous Diseases in Traditional Chinese Orthopedics

    Abstract: Background Establishing the advantageous diseases in the field of traditional Chinese orthopedics treated with manual therapy will promote the development of manual techniques and dominant discipline. However,the current evaluation and selection lack a quantitative evaluation process based on evidence-based medicine. Objective To systematically review systematic reviews of diseases in the field of traditional Chinese orthopedics treated with manual therapy using evidence mapping methodology and explore the advantageous diseases in this field. Methods Computerized searches were conducted in PubMed, EMbase,The Cochrane Library,Web of Science,SinoMed,China National Knowledge Infrastructure,Wanfang Data,and VIP database to collect systematic reviews of diseases in the field of traditional Chinese orthopedics treated with manual therapy from inception to March 5,2023. The evidence distribution characteristics were presented using a combination of charts and text. Results A total of 126 systematic reviews published from 2003 to 2023 were included,showing an overall increasing trend in the quantity of relevant studies both domestically and internationally. Methodological quality assessment results indicated that 13 reviews were of moderate quality,64 were of low quality,and 49 were of very low quality. The evidence map showed that the research evidence in the field of traditional Chinese orthopedics treated with manual therapy mainly focused on 18 clinical diseases,including cervical spondylosis,low back pain,knee osteoarthritis,lumbar disc herniation,ankle sprain,adhesive capsulitis of the shoulder,cervicogenic headache,atlantoaxial subluxation,distal radius fracture,lateral epicondylitis of the humerus,carpal tunnel syndrome,lumbar sprain,scoliosis,hip osteoarthritis,fibromyalgia syndrome,myofascial pain syndrome,rotator cuff injury,and supracondylar fracture of the humerus,demonstrating beneficial or potentially beneficial effects. Conclusion Manual therapy is widely used in the clinical practice of traditional Chinese orthopedics. However,due to methodological shortcomings and a lack of research evidence on safety and cost-effectiveness,future efforts should focus on multi-level inter-agency cooperation to establish sound evaluation standards and systems,improve research quality,update research evidence,and further explore the advantages of manual therapy in the field of traditional Chinese orthopedics.

  • Research on Evaluation Index System of Scientific and Technological Achievements in Traditional Chinese Medicine Clinical Research

    Abstract: Background  The effective evaluation of scientific and technological achievements in traditional Chinese medicine(TCM) is of great significance in promoting the innovative development of TCM. Previous evaluations of scientific and technological achievements in TCM often used a generalized evaluation method,leading to problems such as inappropriate indicators and limited dimensions,which is not conducive to reflecting the multiple values of achievements in a scientific and comprehensive way. Consequently,there is an urgent need to establish categorized and multi-dimensional evaluation index system for the evaluation of scientific and technological achievements in TCM in the new period. Objective  To establish a scientific,categorized,reliable and comprehensive evaluation index system that reflects the principles of TCM and highlights its unique advantages aiming at the scientific and technological achievements in TCM,in accordance with the contemporary requirements for scientific and technological evaluation in China. Methods  This study began in November 2022 and was completed in May 2023. A preliminary index framework was developed through literature review and focus group discussions. Delphi expert consultation was then conducted over two rounds to determine the evaluation indexes and points. The hierarchical analysis method was used to calculate the weight of each index. Results  The final evaluation index system of scientific and technological achievements in TCM clinical research comprised 10 threshold indicators,4 primary indicators,17 secondary indicators,and 17 evaluation points. After testing,the positive coefficient of experts in both rounds of Delphi method implementation was 100%,the authority coefficient was 0.94 and 0.93,respectively. The Kendall's W consistency test for each level of indicators yielded two-tailed significance(P<0.05),indicated high levels of expert positivity and authority,strong consistency of expert opinion,and reliable consulting results. Conclusion  This study constructs a scientific,reasonable,reliable and easy-to-operate evaluation index system for scientific and technological achievements in TCM clinical research. The indexes cover a wide range with highly differentiated weights and TCM characteristics,which is conducive to highlighting the core strengths while comprehensively considering the multiple value of the results. Additionally,the associated evaluation points,enhancing the operationalization of the index system,may potentially serve as a valuable reference for related evaluation activities.

    Abstract: Lumbar spinal stenosis(LSS) is a common disease in orthopedics. The American Society of Pain and Neuroscience(ASPN) published a new version of minimally invasive treatment practice guidelines in 2022 to improve the clinical efficacy of LSS. A total of seven treatments for LSS were included in the guidelines,including percutaneous image-guided lumbar decompression(PILD),interspinous spacers(ISP),interspinous fusion(ISF),intrathecal drug delivery systems(IDDS),spinal cord stimulation(SCS),epidural steroid injections(ESI)and open decompression. Based on the guidelines,this paper reflects and discusses the research progress of minimally invasive treatment of LSS in the context of the clinical situation,aiming to provide a reference for clinicians and further related researches.

    Abstract: Spinal degenerative diseasesare currently most common clinical conditions and alsodominant diseases in the field of traumatology and orthopedics. Traditional Chinese manipulative therapyis a commonapproach for spinal degenerative diseases in traumatology and orthopedics of Chinese medicine. However,the current researches on the efficacy mechanism of traditional Chinese manipulative therapy is still not deep enough,which hinders the modernization and internationalization of the study on the manipulative therapy. Combination of medicine and industry is a novel research idea generated by the combination of relevant scientific and technological progress and the modernization of traditional Chinese medicine in recent years,which is helpful to realize the modernization of the research on the efficacy mechanism of traditional Chinese manipulative therapy and promote the internationalization of traditional Chinese manipulative therapy. Using the application status of modern engineering technologies such as mechanical engineering technology,software engineering technology,neural engineering technology and artificial intelligence technology in the research of traditional Chinese manipulative therapyas research ideas,this paper sorts out the researches on the modernization on the efficacy mechanism of traditional Chinese manipulation under the "combination of medicine and industry" research model in recent years,in order to provide reference ideas for the subsequent researches of the efficacy mechanism of manipulation in the treatment of spinal degenerative diseases.

  • Current challenges and strategies for coping with multimorbidity in the elderly population

    Abstract: China has entered an aging society, and multimorbidity is a prominent public health problem in the elderly population. It is important to summarize the research in the field of multimorbidity in the elderly population and to analyze and discuss the strategies for the comprehensive clinical management of the elderly multimorbidity population. Herein, we analyzed the current status of research in this field in terms of multimorbidity management practice, multimorbidity diagnosis and treatment model, multimorbidity medication strategy and multimorbidity medical and nursing team building. Meanwhile, we proposed to establish a patient-centered multimorbidity management research program, promote the application of geriatric multimorbidity assessment tools, conduct research on the common etiology and mechanism of co-morbidity, develop standardized guidelines/consensus on multimorbidity management, and draw on the "Holism concept of TCM" of "Treating diseases with syndrome", integrate Internet+ technology and smart wearable devices, and conduct early risk assessment and management of multimorbidity. We provided a comprehensive outlook on late research directions in the field of multimorbidity with the aim of providing a reference for further in-depth research in this field.

  • 北京市社区老年人群维生素D水平调查及其与生命质量的相关性研究

    Abstract: Objective To investigate the vitamin D level of the elderly in Beijing communities and its correlation with health-related quality of life, so as to provide decision-making reference for improving the quality of life of this population. Methods From November 2017 to July 2018, clinical information and biological samples of residents were collected in 10 communities in Chaoyang District and Fengtai District, Beijing, and the EQ-5D scale was used to evaluate the quality of life of the subjects. According to the 25(OH)D level, 1066 elderly subjects were divided into vitamin D deficiency group, insufficient group and sufficient group, and statistical analysis was performed. Results There were 798 people with vitamin D deficiency, accounting for 74.86%; 240 people with vitamin D deficiency, accounting for 22.51%; only 2.63% of subjects had sufficient vitamin D and no vitamin D excess. There were 326 (40.85%) and 80 (33.33%) patients with osteoporosis in the vitamin D deficiency group and the insufficient group, and 9 (32.14%) in the vitamin D sufficient group. In terms of bone mineral density, the overall T value of the left hip, the overall T value of the right hip, and the overall T value of the hip were statistically significant between groups with different vitamin D content (P<0.05); in terms of grip strength test, the subject The grip strength decreased with the decrease of vitamin D content in the body, and the difference between the groups was statistically significant (P<0.05). In terms of health-related quality of life, subjects in the vitamin D sufficient group performed the best in the "pain and discomfort" dimension and the EQ-5D total utility value. The difference between the groups was statistically significant (P<0.05), and there was no difference in the other dimensions. See statistical significance. Conclusion The lack or deficiency of vitamin D among the elderly in Beijing communities cannot be ignored. The level of vitamin D is positively correlated with the bone mineral density, grip strength and the total utility value of EQ-5D in the elderly. Early screening of vitamin D levels in the elderly and timely supplementation of adequate intake are of great significance for maintaining and improving the health-related quality of life of this population.

  • 重视骨质疏松症的共病研究与早期筛查

    Abstract:

    The Patients with osteoporosis (OP) often suffer from a variety of comorbidities, including endocrine, circulatory, respiratory, urinary, immune, skeletal muscle, nerve and other multi system diseases. The incidence of OP comorbidity is high, and these comorbidities may aggravate osteoporosis and increase the risk of osteoporotic fracture, seriously affect the quality of patient's life, and make the clinical management more complex. So the situation brings a heavy burden to the families and society. At present, there is a lack of overall research on osteoporosis and its comorbidities. The existing research strategies are difficult to effectively guide clinicians to carry out comorbidity management in terms of common causes, common prevention and common treatment. In view of this, it is suggested to introduce the concept of multidisciplinary integrated treatment (MDT), and strengthen the understanding of osteoporosis related comorbidities and their pathogenesis. Patients who may be complicated with osteoporosis should be screened as early as possible. Once osteopenia and osteoporosis are found, active prevention and intervention should be carried out to reduce the risk of fracture. Early screening, early diagnosis and early treatment is necessary to realize the prevention and treatment of OP comorbidities.
